Gynecomastia – Normal

The physiological gynecomastia is one that is not considered a disease, and is considered normal at a certain stage of life. What is important is that it tends to have spontaneous resolution within one year, and can be divided into neonatal (newborn), pubertal (adolescents) and elderly (>65 years).

Gynecomastia – Permanent change

Most of the time, the cause of pathological gynecomastia is unknown (idiopathic). The exams in these cases are usually all normal, the only alteration being the increase in the volume of one or both breasts, with the breast gland in the retroareolar region being palpated in the physical examination (behind the areola).

Among the known causes, one of the most common is the chronic use of certain medications. The list of medications that can cause gynecomastia is immense. In young adults a common cause associated with increased volume of the mammary gland is the use of anabolic steroids, used to gain muscle mass in gym goers. The drug use, such as marijuana, and the alcohol abuse are also other possible causes.

Hormonal changes such as those caused by doençasthyroid and pituitary diseases podemcan also cause gynecomastia, as well as  >renal failure and a cirrhosis. A rare cause of gynecomastia is testicular or adrenal tumors. Hereditary conditions, such as Klinefelter’s Syndrome, can also lead to gynecomastia.

Pseudogynecomastia – false gynecomastia

The increase in breast volume may also be secondary to fat accumulation in the anterior chest region. These cases are called pseudoginecomastia and are more common in patients who are above their ideal weight. This is not considered a true gynecomastia, but the fat accumulation that is treated through the técnicaliposuction technique.

Breast Cancer in Men

Another important fact to be said is that gynecomastia is not directly related to breast cancer, that is, it does not predispose to the development of cancer. A rare exception is patients with a syndrome of unusual genetic origin called Klinefelter’s Syndrome.
